Payment and Fees

Payment is expected at the time of services. We accept checks and credit cards. Your doctor will provide you with a receipt of payment with the necessary information for you to be able to submit the receipt to your insurance should you choose.

As a “Non-participating Provider” your doctor’s fees may be reimbursed in whole or part by your insurance company.

Other charges could be incurred at an hourly rate for paperwork needing completion by the doctor, copying of chart contents, or extended phone calls.

Nonpayment of bills is grounds for termination of services.  Unpaid bills will be pursued through collections and will incur a service fee.
